The Form below is the application. You are still required to submit your payment to the Youth Director before your child can participate in any youth related activities.
After submitting the form above please print out the form below and fill it out. Then turn it into the Youth Director with your payment. If this is not done you will not be able to participate in Youth Activities or go on any trips.
